Service Member Plaque

Information Required

Please provide the following when placing your order:

• A clear, recent photo of the person
• Full name exactly as it should appear
• Rate and rank
• Command emblem, logo, or another image you would like displayed on the plaque
• Any additional details requested for the design

Photo Requirements

Your reference photo must be clear, well-lit, and high quality. The person’s face should be fully visible and not covered by sunglasses, shadows, or other objects.

Please do not submit photos with beauty filters, heavy editing, screenshots, blurry images, or low-resolution images. Poor-quality reference photos may delay the design and overall processing time while a replacement photo is requested.

Your order will not begin processing until all required information and acceptable reference images have been received. Please carefully check the spelling of all names, rate, rank, and command information before submitting. Custom products are created using the information provided by the customer.
